We provide in-home support services, community coaching, and respite care. As an employee of Kahak Health Care Services (KHCS) you will be a part of a team of skilled and dedicated professionals who value integrity, respect, and teamwork. We are recruiting one RN and one LPN for our office in Indianapolis. We are looking for compassionate and capable professionals dedicated to the care of the individuals we serve. Duties of the RN Position: *These duties are not exhaustive You will be responsible for ensuring quality patient care through assessment, planning, interventions, and evaluation of patients needs as well as Developing nursing care plans and patient protocols Taking and ensuring effectiveness measures Providing services and/or treatments requiring substantial and specialized nursing skill Educating and counseling patients and caregivers in meeting nursing and related needs. Initiating appropriate preventative and rehabilitative nursing procedures and provides referrals to other services and disciplines as needed. Training staff for Medication Administration Certification and MAR Accurately and completely prepares all clinincal documentation and submits within the established timeframe per agancy policy and state regulations Participating in personal and professional growth and development and maintains current knowledge of industry guidelines, clinical skills and competency Supervising LPN staff ***Administration Train-the-Trainer certification is required.*** Duties of the LPN Position: *These duties are not exhaustive Administer medications and injections as prescripbed by healthcare providers in accordance with KHCS policies and procedures Perform routine nursing procedures Collaborate with KHCS healthcare professionals to develope and implement patient care plans Document patient information accurately and timely in electronic medical records using KHCS designated systems Respond effectively to emergencies and provide basic life support, educate patients and caregivers about healthcare prodeures, medications, and self-care techniques, and communicate effectively with KHCS staff, individuals, and caregivers to ensure coordinated care delivery Participate in quality improvment initiatives at KHCS Stay updated on current healthcare practices and regulations through continuing education Assist in the admission, transfer, and discharge of patients Both positions require: A minimum of one year experience A reliable car A current driver's license A current work authorization (US Passport, US Work Authorization card, Permanent Resident Card) or a Birth Certificate Social Security Card Current auto insurance Current CPR & First Aid Current physical exam Current tuberculosis test with negative result Resume If hired, background checks (FBI) and a Central Registry (CRS) check will be required.
