Registered Nurse (Telephonic RN Care Coordinator) Remote | Full-Time | $80,000–$100,000 based on experience and location ilumed – Value-Based Care / ACO REACH & MSSP ilumed is seeking a Registered Nurse to join our remote Care Management team. In this role, you will provide telephonic care coordination, disease management support, and patient education to Medicare beneficiaries, helping them navigate their health with confidence while improving clinical outcomes. What You’ll Do Provide telephonic care guidance, education, and support to beneficiaries and caregivers Collaborate with physicians and care teams to develop integrated care plans Use motivational interviewing to engage patients and identify care needs Meet care management KPIs for call volumes, caseload, and documentation Identify SDOH needs and connect patients with appropriate resources Ensure compliance with HIPAA and regulatory standards What You Bring This role requires a Compact RN License as well as California and New York state RN licenses. If you don’t have one or more of these yet, that’s ok —we’re happy to cover the licensing costs for the right candidate.
