 What You Should Know About the RN Care Manager Role: * This is a full-time RN Care Manager position serving Zanesville and surrounding areas.   * Schedule: 8:00am - 4:30pm with weekend and holiday rotation. * The RN Float is a DRIVING position The RN Care Manager's Essential Duties Are: The RN Care Manager carries the responsibility of flexibility in diverse Ohio’s Hospice RN roles as a float team member assisting Care Partners, Coming Home, Extended-Hours/On-Call and the Admission Departments and IPU as needed.  Float team staff will collaborate with various IDT members both internal and external of the organization.  Clinical services will be scheduled in a timely fashion according to the patient’s plan of care, observing patient’s continuity of care to achieve the desired outcome of comfortable and peaceful death for patients. When in the field, staff will provide excellent clinical care to all Ohio’s Hospice patients that meet the needs of our patients/families, Ohio’s Hospice standards, Medicare Conditions of Participation and achieve the desired outcome of comfortable and peaceful death for our patients.
