Our Core Values Quality Safety Compassion Integrity Collaboration Community The RN Care Manager Role As an RN Care Manager at PACE North, you’ll be more than a nurse - you’ll be a decision maker, advocate, and trusted partner in care. In this role, you will: Serve as a core member of the interdisciplinary team, shaping care plans and ensuring participants’ needs are met. Collaborate with staff, participants, and families to build strong, supportive relationships. Conduct initial and periodic health assessments. Monitor and coordinate supplies, equipment, and services. Visit participant homes and participate in the on-call rotation. Lead care transitions, including hospital-to-home discharge planning. Provide education, advocacy, and leadership that empowers participants to live their best lives. This is a full-time, in-person position (not remote).
