Manages the nursing care for an assigned caseload of approximately 15 patients with varying acuity and needs. Caseloads above this target will be co-managed or additional support will be offered. In the event the assigned caseload falls below this target, the nurse may be asked to provide support to other team members. ESSENTIAL DUTIES & RESPONSIBILITIES: * Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation * Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens. * Record and report the results of the nursing assessment. * Plan, initiate, deliver, and evaluate appropriate nursing acts. * Teach, assign, or supervise other personnel in implementing the treatment regimen. * Collaborate with other health care providers in determine the appropriate patient and family-centered plan. * Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen. * Provide teaching and counseling about the patient's health. * Report and record the plan for care, nursing care given, and the patient's response to that care. * Supervise, teach, and evaluate those who perform nursing functions and administer nursing programs and nursing services. * Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly. Responsibilities

Coordinates and delivers nursing care for a caseload of approximately 15 hospice patients while supervising LPN and Hospice Aide staff. Collaborates with healthcare providers to implement and evaluate patient- and family-centered treatment plans.
