Job Details Level: Entry Job Location: El Paso Childrens Hospital - El Paso, TX 79905 Position Type: Full Time Education Level: 4 Year Degree Job Shift: Day Job Category: Health Care Comprehensively plans for case management services for a targeted patient population. Carries out activities related to utilization management, discharge planning, care coordination and referral to other levels of care. Works with physicians, Social Services, and interdisciplinary team to facilitate clinical pathways to achieve desired treatment outcomes. Promotes interdisciplinary collaboration and champions service excellence. Works collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum at the appropriate level of care. Coordinates and oversees the care and service of patients to achieve desired clinical and resource outcomes. Assesses need for continued care for patients, applies utilization review concepts according to federal, state, and JCAHO guidelines. Qualifications Work Experience: 5 years' experience in healthcare or a related field required, of which 3 years are in management/supervisory position. License/Registration/Certification: Current Texas RN License or Multistate RN License. Current Basic Life Support (BLS) Education and Training: Bachelor's degree in healthcare or a business-related field required. Skills: Must be able to demonstrate initiative to work in a fast-paced environment with demonstrated ability to manage multiple tasks and demands. Excellent interpersonal skills. Excellent organizational skills. Excellent written and oral communication skills. Knowledge of Hospital setting. Must possess analytical, problem solving and decision-making skills. Competent in Microsoft Office products (i.e. Word, Power Point and Excel). Ability to work independently with minimal supervision. Demonstrated project and systems management skills. Bilingual English/Spanish preferred.
