Responsibilities: · Plans, coordinates, implements, and evaluates the nursing care of all patients. · Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs · Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients. Requirements · Must be at least 21 years of age. · A valid non-restricted RN license in the state of Colorado is required. · Must have a valid non-restricted driver’s license. · Flexibility and availability to work a weekend schedule if needed. · At least 1 year of home health or hospice experience preferred.

Responsibilities

The Registered Nurse will plan, coordinate, implement, and evaluate the nursing care for all assigned patients. This involves utilizing clinical knowledge to identify patient needs and coordinating necessary services and referrals while working closely with the interdisciplinary team to maintain a responsive plan of care.
