The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care. RN ED CM I requires six (6) months of recent full-time acute care experience in an acute care hospital. RN ED CM II requires one (1) year of case management experience. Qualifications Notice to Applicants:The position of RN ED Case Manager is open at two (2) tiers: I and II. Placement into a specific tier will be determined based on the candidate’s qualifications, experience, and skills. The pay for this role will correspond to the assigned tier and ranges from $52.93 to $81.58 per hour, depending on qualifications and years of experience. Minimum Qualifications: Graduate of an accredited school of registered nursing Current CA Registered Nurse (RN) License RN ED CM I requires six (6) months of recent, full-time acute care experience in an acute care hospital RN ED CM II requires one (1) year of case management experience Preferred Qualifications: Bachelors of Science in Nursing (BSN) Bilingual in Spanish One (1) year of experience in Case Management and strong discharge planning skills One (1) year of experience with InterQual One (1) year of experience with Allscripts and Meditech Essential Functions and Responsibilities: Acts as a liaison between physicians, nursing, other hospital departments, community agencies, health p.ans and patients/families. Responsible for discharge and transitional care planning. Identifies and assures patient continuum of care needs are met. Involves and educates patient/families regarding discharge planning and community resources. Collaborates with appropriate community resources on behalf of the patient. Refers to Social Services when appropriate. Refers quality of care issues according to policy and procedure. Reports suspected or actual abuse. Participates in Process Improvement activities.
