Job Title: Registered Nurse Case Manager Job Overview: As RN Case Manager of Luminos Hospice, you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best-in-class hospice care. Responsibilities Coordinates and oversees all aspects of patients' home care. Collaborates with patient care teams to develop and implement care plans. Educates families on patient condition, needs and care. Performs head to toe physical assessments. Checks vital signs and manages medications. Administers and maintains catheters. Complete documentation timely and accurately. Supervises HHAs. Regularly communicates patient progress to the clinical manager, physician and care team. Other duties as assigned.
