QUALIFICATIONS:

  • Licensed as a Registered Nurse / RN in the state you work in.
  • Proficiency with Electronic Medical Records (EMR) software.
  • Excellent planning, organizing, and communication skills.
  • Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
  • Valid Driver’s License and reliable vehicle with current insurance.
Responsibilities
  • Conduct initial client assessments and continuously refine care plans.
  • Provide hands-on nursing care and patient education.
  • Maintain open lines of communication with patients, families, and the healthcare team.
  • Manage patient care coordination for your caseload.
  • Participate in interdisciplinary team meetings to ensure comprehensive care planning.
