This position will be an integral part of the Primary Care Health Team within La Ronge Medical Clinic (LRMC). Although the primary base of work will be at the medical clinic, some travel will be required to the outpost health clinics as part of the Primary Care Health team.. The focus of the RN Case Manager will be to offer patient case management through holistic care coordination within a multidisciplinary health care team. The RN Case Manager will take a lead in fostering close therapeutic relationships between the primary care customer, their families, and the wider clinical team primarily through disease management and acute conditions which involve assessment, diagnosis, referral, health teaching, advocacy, and health promotion. There will be limited transfer of medical function (ToMF), which will advance to the Authorized Specialty Practice through expanded training as set out by the College of Registered Nurses of Saskatchewan (CRNS). The RN Case Manager will be working to their full scope of practice as established by the CRNS.
