REQUIRED QUALIFICATIONS

• Associate’s Degree in Nursing
• Active Georgia RN License (Single or Compact)
• BLS Certification (Instructor-level accepted)
• Minimum 1 year of acute care nursing experience

SKILLS NEEDED

• Excellent communication and collaboration skills
• Strong clinical judgment and coordination experience
• Knowledge of discharge planning and community-based resources
• Comfortable in a fast-paced hospital setting

ABOUT THE ROLE

We’re hiring a Care Coordinator RN / RN Case Manager to support our Care Coordination team in Marietta, GA. You’ll play a key role in leading care transitions, discharge planning, and care progression for acute care patients—from admission to safe discharge.

WHAT YOU’LL DO

• Assess patient needs, including chronic disease and psychosocial risks
• Collaborate with care teams, patients, and families on care/discharge plans
• Identify and help resolve delays in treatment or discharge
• Coordinate post-acute care and safe transitions
• Attend rounds and contribute to interdisciplinary care planning
• Document all assessments, interventions, and outcomes accurately
