REGISTERED NURSE CASE MANAGER    Are you passionate about helping those who struggle to access care or feel unwelcome in other healthcare spaces?   Western North Carolina Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC) serving underserved communities in Western North Carolina.   WNCCHS is seeking a mission-minded Registered Nurse Case Manager to provide care management to patients with Medicare and Medicaid at our Minnie Jones Health Center in Asheville.   RN Case Managers collaborate with medical, behavioral health, pharmacy, dental, and care management teams to provide customized care plans to improve health outcomes.
