EDUCATION, QUALIFICATIONS, CREDENTIALS: Current Indiana license as a Registered Nurse. Graduate of an accredited school of nursing. Minimum of one year in nursing management in long-term care. Two years professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting

The RN Clinical Navigator/MDS Staff at Aspen Trace plays a crucial role in ensuring that residents receive comprehensive and coordinated care throughout their stay. This position involves assessing patient needs, developing care plans, and advocating for patients to ensure their rights and preferences are respected. The RN will collaborate with interdisciplinary teams to facilitate smooth transitions and optimize patient outcomes. Additionally, the role includes conducting Minimum Data Set (MDS) assessments to ensure compliance with regulatory standards and to support quality improvement initiatives. Ultimately, the RN Clinical Navigator/MDS Staff is dedicated to enhancing the quality of life for residents through effective care management and patient advocacy.
