JOB RESPONSIBILITIES : Collaborates with dialysis providers to ensure communication between facility and dialysis centers is consistent, accurate, and information is followed upon Responds to resident concerns regarding dialysis care Monitors the process of obtaining accurate weights of dialysis residents Trains licensed nurses on assessment skills for monitoring dialysis residents and monitors licensed nurse alert charting compliance Ensure staff are competent with dialysis care, vascular access devices, fluid and dietary restrictions, etc Communicate with medical practitioners and other clinical managers to effectively problem solve and manage the facility dialysis population Participate/lead quarterly reviews of resident outcomes, dialysis efficiency, anemia management, and overall dialysis program success QUALIFICATIONS & EXPERIENCE REQUIREMENTS: Registered Nurse in the state of Virginia Registered Nurse with at least 5 years medical/surgical experience Previous Long Term Care Experience THE COMMUNICARE COMMITMENT First and foremost, we are a compassionate, family-owned company who truly believes in the care we provide. Responsibilities
