This is a flex nursing role which provides case management, home health and clinic visits in Napa county. Schedule: Weekdays Monday to Friday 8am-4:30pm. No weekends or holidays! As a member of the Interdisciplinary Team, the Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to our frail elderly participants.
