In this role you will be responsible for: Clinical Care: Performs physical assessments, wound care, medication management, and symptom control (e.g., managing pain). Case Management: Develops, implements, and updates individualized care plans while tracking patient progress. Coordination: Acts as the main point of contact for the IDT, including doctors, social workers, and chaplains, to ensure cohesive care. Education & Support: Educates patients and families on the disease process, medication, and caregiving techniques. Administration: Handles documentation, admission paperwork, and supervises Hospice Aides. Job Summary: Provides operational assistance to the daily case management activities. Provides ongoing education, coaching, and mentoring to assigned staff. Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Monitors reports as outlined by department leader to ensure daily workflow/volumes are managed, opportunities are identified, and interventional results are realized and acted upon. May post metrics and progress data at the local level. Works in collaboration with the department leader in oversight of day-to-day operations and problem solving. Identifies and gathers pertinent performance/quality concerns and addresses them as appropriate. Participates in the performance review process by soliciting peer feedback, collating on behalf of department leader, and providing input. Assists with oversight and supervision of staff to include staff planning/attendance, management/recording and time off process. Assists department leaders to ensure compliance/completion of assigned/required tasks and departmental policies/procedures. Participates in the development of training and procedure manuals. Leads or participates in regular staff and department meetings. Ensures compliance with Condition of Participation (CoP) discharge planning. Identifies and escalates gaps/barriers related to inpatient and observation length of stay. Assesses patients at high risk for readmission. Identifies root cause for readmission. Develops plan and collaborates with multidisciplinary team regarding discharge planning to next level of care. May provide input for hiring, training, leading, and evaluating staff. Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient’s age-specific needs and clinical needs as described in the department's scope of service. Works in a constant state of alertness and safe manner. Performs other duties as assigned. EDUCATION Graduate of accredited school of nursing or education equivalency for licensing EXPERIENCE Three years' care coordination experience PHYSICAL REQUIREMENTS Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. Frequent keyboard use/data entry. Occasional bending, stooping, kneeling, squatting, twisting and gripping. Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. Rare climbing.
