The RN/LPN Extended Hours Essential Duties Are:  Plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.     What You Should Know About the RN/LPN Extended Hours Role:   * This is a DRIVING position throughout Wooster * This is a Full-Time position: * 4p-12a three days a week and 8a-8p every other weekend.  * We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed. RN/LPN Extended Hours Qualifications:   * Graduate of an accredited nursing school; Bachelor of Science in Nursing (BSN) preferred.  Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred. * CPR Certified  * Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification. * Computer skills sufficient to properly document services and care.   * Ability to drive during daytime, nighttime, or inclement weather. * Valid Driver's License with Safe Driving Record   * State Minimum Automobile Insurance Coverage  * Must be able to pass a criminal background check  * Must be able to pass a 10 panel drug screen LPN/RN Extended Hours Qualifications:   * Graduate of an accredited nursing school.  Active licensed practical nurse in the State of Ohio without any board actions.
