RN/LPN (Licensed Practical Nurse) - Supervisory Responsible for the health and welfare of the people supported in the medical residential group home and providing a family-type home atmosphere. Manages all activities including upkeep of home and food budgeting. LPN staff at Sertoma have an exceptional nurse to patient ratio of 3:1 and are assigned a full-time Direct Support Professional as an assistant. Shift: 12-hour, day shift *Reports to: Agency Chief Operations Officer for clinical related issues and Residential Directors and Residential Coordinators for residential related issues *Supervises: Direct Support Staff working in the medical residential home. *Works with: Individuals we serve, human resources, case managers, clinic staff, residential staff, employment staff *External Relationships: TNDDA, Managed Care Organizations, family/guardians/conservators, employers, community partners, *Independent Support Coordinators, Support Coordinators (ECF), physicians, pharmacy Qualifications: *Current unencumbered RN/LPN license *Work Experience: Previous experience with ID/DD population preferred but not required. Time management skills and ability to multi-task are beneficial in this position *Physical Requirements: Sight and hearing, ability to lift/assist people supported and to intervene during behavioral episodes in a manor in keeping with company policies Responsibilities: 1. Ensure health and safety of people supported by ensuring adequate exercise, providing assigned nursing services, maintaining up to date postings of medical/emergency information, ensuring home is hazard free, participating routinely in training for fire evacuation, maintaining clean/sanitary environment and reporting any maintenance needs promptly. 2. Ensure a family atmosphere through interaction, counseling, outings, conversation, etc. 3. Meet the needs of the people supported through training and maintaining a clean home, meal preparation, assisting with personal hygiene, and shopping. 4. Conduct and participate in monthly house meetings. 5. Encourage people supported to follow dietary menus. Post all menus/special diets for easy access for other staff. 6. Plan and follow through with regularly scheduled recreational activities at the house. 7. Maintain house records in an orderly and up to date manner (i.e., fire drills, med requests, on-site person supported records, licensure, ISP, etc.) 8. Work regular schedule, participate as a team player, offer to cover, or swap shifts when time off is needed. Sertoma nurses are required to cover shifts with existing nursing staff. 9. Distribute medications and other medical treatments as prescribed. Follow nursing plan of care. Maintain daily medication counts and reorder medications as necessary according to agency policy and procedures. Report medication errors when discovered. 10. Attend circle of support meetings, monthly staff meetings and in-service meetings, ISP meetings. Provide input to the RN, case manager and residential department on all plans. 11. Maintain the vehicle logs to include regular cleaning, monthly mileage form, gas receipts etc. Promptly report any vehicle problems to the maintenance department via work order or phone call to maintenance department. 12. Submit monthly paperwork to the residential department according to schedule as assigned. 13. Familiarize self with licensure, fire marshal and quality assurance standards and always maintain group home within these standards. Meet with residential director or residential coordinator regarding any issues. 14. Communicate with the appropriate staff immediately regarding any concerns, problems, or outside issues from external relationships. 15. Complete and document nursing notes daily.
