Description Job Summary: Ensure and enhance the health and well-being of our Residents by planning and providing direct care and overseeing the activities of personnel as the “person in charge” according to the Nurse Practice Act. Reports to: Director of Nursing Nursing Care Functions · Maintains competence in all required skills and completes continuing education as assigned · Makes rounds to residents to observe physical and psychosocial status · Performs nursing techniques for the comfort and well-being of the residents, educating and allowing choices with respect to resident rights · Administers prescribed medications and treatments per physician orders · Is knowledgeable about various medications and reports resident response and side effects, responds to drug considerations and reports problems or potential problems to care team · Assesses, reports, and documents any change of condition · Follows all shift routines, policy and procedures regarding monitoring of vital signs, weights, administration of medications and treatments · Keeps families updated on resident condition · Follows all Advanced Directive protocols and orders · Follows all Infection Prevention and Control policies and procedures · Immediately reports any suspected abuse or neglect per company policy Personnel Functions · Provides employee performance feedback to the supervisor · Makes recommendations to the supervisor concerning employee training and counseling · Attends nursing meetings as scheduled · Develops and maintains a good working relationship with other staff, whether within the nursing department or with inter-departmental personnel · Utilizes materials and supplies in an efficient, cost-effective manner · Follows all facility policies and procedures, standard precautions, and works within OSHA guidelines · Documents and reports all incidents or accidents that occur involving residents, visitors, or staff · Reports any equipment malfunction to the designated staff member · Remains free of criminal conviction that may preclude employment in the nursing home setting (including but not limited to theft, battery, assault) · Reports any pending criminal or civil charges or matters to the Administrator by the next business day · Always represents the United Pioneer Home in a positive manner unless contraindicated by law · Maintains a clean, safe and efficient work environment · May participate in various committees and meetings of the facility to enhance interdisciplinary process Charting, Documentation and Assessment Functions · Contributes toward completing and updating resident’s care plans · Accurately transcribes physician’s orders and forward necessary information to other departments · Charts nurses’ notes in an informative and descriptive manner that reflects the care provided as well as the resident response · Provides and documents ongoing resident assessments Charge Nurse Functions · Provides leadership and direction on the unit, acting as a role model for CNAs and other staff · Listens to shift report and keeps open lines of communication with supervisor, co-workers and oncoming shift · Ensures that unit personnel are carrying out their assignments · Reports occupational exposures to needlesticks, blood, body fluids, infections materials, and hazardous chemicals in accordance with facility policy and procedures · Initiates disciplinary action/coaching when the performance of personnel warrants such action · Assigns personnel appropriately to meet resident needs · Assists with replacing staff after call-ins or unforeseen changes in staffing of resident care needs Requirements Qualifications · Current licensure in the State of Wisconsin as a Registered Nurse or Licensed Practical Nurse · Current CPR registration · Previous experience or aptitude for working in the long-term care environment Other Skills and Abilities required · Demonstrated concern for the elderly and ability to treat Residents with respect and dignity · Clinical competence to utilize the nursing process to perform physical and cognitive assessments · Good problem-solving skills with ability to make independent judgements utilizing initiative, discretion, and the nursing process · Strong leadership, time management, and teamwork skills · Effective, positive communicator with staff, families and outside agencies and vendors · Ability to plan and prioritize work duties · Customer service oriented Physical Requirements · Ability to endure prolonged walking, sitting, standing, pulling, bending, and stooping movements · Able to lift a minimum of 30 pounds · Be in good general health and demonstrate emotional stability · Achieve negative assessment for infection with Mycobacterium tuberculosis (TB) · Remain free of illegal drug use and signs of impairment as per facility policy · For full-time and part-time staff only: ability to work alternating weekends and adapt to changes in bi-weekly work schedule
