Description Facility: Sylva Valley Health & Rehab (Skilled Nursing Facility) Job Type: Full-Time About the role Sylva Valley Health & Rehab is hiring an experienced RN MDS / Case Management Nurse to lead accurate assessments, strong clinical documentation, and coordinated care in our skilled nursing setting. This role is ideal for a nurse who is detail-oriented, deadline-driven, and confident managing MDS timelines while collaborating with the interdisciplinary team. What we offer Competitive pay (based on experience) Consistent weekday-focused schedule (discussed during interview) Supportive leadership and collaborative IDT culture Vacation time + Sick time (per eligibility) Key responsibilities Coordinate and complete MDS assessments accurately and on time (OBRA/PPS as applicable) Manage ARD scheduling, tracking, and compliance timelines Ensure documentation supports skilled services and reimbursement (PDPM/PPS focus) Participate in/lead IDT and care plan coordination Support case management needs: clinical updates, payer communication, continued stay reviews as needed Collaborate with therapy, nursing, providers, admissions, and leadership to ensure appropriate level of care and accurate documentation Maintain compliance with facility policies and applicable regulations Required qualifications Active Registered Nurse (RN) license MDS experience required in a skilled nursing facility PointClickCare experience required Strong knowledge of MDS processes, care planning, and skilled documentation Excellent organization, communication, and time management skills Preferred (nice to have) Experience with PDPM/Medicare skilled documentation and managed care reviews MDS certification Apply Apply online: tinyurl.com/sylvavalley
