Overview RN Navigator - Cardiovascular Center Waterloo, IA Full-time, 40 hours/week 8 hour shifts The Navigator develops and coordinates the overall plan of care for a cohort of patients throughout their hospital stay and beyond (as directed), establishing strong partnerships with patients, families, physicians and all care givers. Responsibilities The Navigator develops and coordinates the overall plan of care for a cohort of patients throughout their hospital stay and beyond (as directed), establishing strong partnerships with patients, families, physicians and all care givers. Assumes a broad level of accountability for the outcomes of care for assigned patients and provides leadership to the multi-disciplinary team. Qualifications Current license in the state of Iowa or meets criteria established by Iowa Board of Nursing. Certification in specialty preferred; commitment to obtain within 2 years of hire date. BSN required or actively enrolled in BSN completion program. MSN preferred. 3 years recent experience in direct patient care involving specialty. Team leadership skills and excellent communication skills. Able to meet the physical demands on file in the Human Resources Department.
Responsibilities
The RN Navigator develops and coordinates comprehensive care plans for cardiovascular patients throughout their hospital stay and beyond. They provide leadership to the multi-disciplinary team and ensure accountability for patient outcomes.
