Position Summary We are seeking a RN Nurse Navigator for our prostate cancer clinic which provides patient-centered guidance and clinical support to individuals diagnosed with prostate cancer. The ideal candidate will be a compassionate, detail-oriented individual with a commitment to patient care, teamwork, and clinical excellence. Responsibilities Serve as the primary point of contact for newly diagnosed prostate cancer patients, initiating navigation at diagnosis Provide individualized education on prostate cancer, treatment options, side effects, and quality-of-life considerations Assess and address psychosocial, financial, and logistical barriers to care Coordinate referrals, scheduling, and follow-up across urology, radiation oncology, medical oncology, and ancillary services Act as a liaison among patients, providers, and multidisciplinary care teams to ensure seamless care transitions Connect patients with internal and community resources, including financial counseling, support services, and survivorship programs Utilize data mining, reporting tools, and specialty software to track patient pathways, outcomes, and quality metrics and support performance improvement initiatives Identify trends and barriers to timely care and collaborate with leadership on process improvement efforts Support patient satisfaction initiatives and promote an exceptional patient experience Travel to clinics throughout the Charlotte region to support APC clinics and patient navigation needs Requirements Qualifications Active Registered Nurse (RN) license in North Carolina Bachelor of Science in Nursing (BSN) preferred Clinical nursing experience required; experience in urology, oncology, surgical services, or care coordination strongly preferred Knowledge of prostate cancer disease processes, treatment pathways, and survivorship issues preferred Strong communication, organizational, and patient advocacy skills Ability to address sensitive and personal patient concerns with empathy and discretion Proficiency with electronic medical records (EMR) systems Requires travel among some Charlotte sites Why Join Us?
