New Age Hospice is seeking a LPN/RN for Hospice Clinical Liaison Hospice Clinical Liaison Location: El Paso, TX The Hospice Clinical Liaison serves as a key member of the hospice care team, responsible for building relationships with referral sources, educating the community on hospice services, and facilitating smooth transitions for patients into hospice care. This role combines clinical expertise with outreach and communication skills to ensure patients and families receive timely, compassionate, and appropriate end-of-life care. Essential Duties and Responsibilities: Serve as the primary point of contact for hospitals, skilled nursing facilities, physicians, and other referral sources. Conduct clinical assessments to determine hospice eligibility in collaboration with the hospice medical director and interdisciplinary team. Educate patients, families, and healthcare providers about hospice philosophy, services, and benefits. Coordinate and expedite the hospice admission process, ensuring a seamless transition of care. Maintain accurate documentation and communication in accordance with regulatory and organizational standards. Develop and maintain strong relationships with referral sources to increase awareness and utilization of hospice services. Participate in community outreach events and educational programs as a hospice ambassador. Collaborate with the marketing and operations teams to support census growth and strategic initiatives. Qualifications: Registered Nurse (RN) or Licensed Practical Nurse (LPN) required; Current state nursing license in good standing. Preferred 2 years of clinical experience in hospice, palliative care, or related healthcare setting. Experience in sales, outreach, or liaison roles preferred. Skills: Strong interpersonal and communication skills Ability to work independently and collaboratively Knowledge of Medicare hospice benefit and regulatory guidelines Compassionate, patient-centered approach Proficient in EMR systems and Microsoft Office Suite Working Conditions: Frequent travel to hospitals, facilities, and patient homes. May require evening or weekend availability based on referral needs. Hybrid or field-based role depending on organizational structure.
