DESCRIPTION

$7,500 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.
The RN provides care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues. Population-based disease management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient centered PACE interdisciplinary team (IDT).

REQUIRED QUALIFICATIONS:

• Upon hire: Oregon Registered Nurse License
• Within 30 days of hire: National Provider BLS - American Heart Association
• 2 years of RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience.

PREFERRED QUALIFICATIONS:

• Bachelor’s Degree in Nursing.
• Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice
