Description University Health Network (UHN) is hiring a Registered Nurse Patient Care Coordinator to provide telephonic outreach to identified patients and/or family/caregiver. The position requires normal business hours Monday-Friday and offers a work from home option for employees who meet performance requirements. Candidate must reside in East Tennessee region and be able to commute to the corporate office located in Knoxville, TN for occasional meetings. Candidate must be able to maintain HIPAA privacy requirements when working in a home office setting. Note: This position is funded through a grant and is expected to continue until December 31, 2026. The Patient Care Coordinator is responsible for engaging, cultivating, and supporting primary care and specialty providers co-management with timely communication, inquiry, follow-up, and integration of information regarding transitions of care and referrals. The key focus of this outreach is to improve the health of patients and reduce health care costs for the managed population of patients. Requirements Qualifications · Licensed Registered Nurse · 3-5 years’ experience in clinical setting · Evidence of essential leadership, communication, education, and counseling skills · Proficiency in communication technologies (email, cell phone, etc.) · Highly organized with ability to keep accurate notes and records. · Care coordination and/or case management experience is desirable. · Experience with health IT systems and reports is desirable. · Local knowledge about and connections to community health care and social welfare resources is desirable.
