We are seeking Registered Nurses (RN) and Registered Practical Nurses (RPN) for opportunities within the community. There are both full-time and part-time positions available across the Toronto Central & Durham regions. all positions require a valid (G2 or G) driver’s license and reliable vehicle.
About VHA Nursing Programs:
The Adult Visiting program provides specialized care services to individuals aged 18 and above who require complex care and specialized nursing skills in a home setting. We are currently hiring for this program in the Toronto Central and York regions.
The Palliative Visiting program delivers home nursing services dedicated to end-of-life care for individuals, families, and groups. This program focuses on effective symptom and pain management to improve their overall quality of life. We are currently hiring in the Durham region.
The Child and Family program is designed to support families caring for children with medically complex needs, spanning an age range from newborn to 25 years. This program offers both shift and visiting nurse opportunities to accommodate the diverse care needs of our clients, families, and groups. We are currently hiring in the Toronto Central, North York, and Scarborough regions.
The Foot Care Nurse program focuses on providing foot care services to clients in community clinics, long-term care facilities, and home based settings. We are hiring RPN who have Foot Care Nurse Certification from a VHA-recognized institution, and who have completed a minimum of 100 hours of experience as a Foot Care Nurse. This program covers the Central West region.
The NSWOC program focuses on prevention and reduction in the incidence of chronic wounds and pressure injuries. A NSWOC provides specialized holistic assessment and management as an inter-professional team member to meet the needs of individuals with ostomies, acute, and chronic wounds. NSWOCs will work in the Toronto region.

REQUIRED QUALIFICATIONS:

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Membership of your Professional Association preferred (RNAO/WeRPN).
  • RPN diploma or RN diploma/degree from a recognized university or college.
  • A valid Ontario driver’s license (G2 or G class) and a reliable vehicle for traveling is required.
  • Valid First Aid or CPR Certification.
  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
Responsibilities

KEY AREAS OF ACCOUNTABILITY

VHA’s Nurses are required to do the following tasks, but not limited to:

  • Assess, Monitor, and Makes Care Planning/Care coordination
  • Evaluates care plan interventions through on-going reassessment and evaluation results.
  • Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
  • Demonstrates professionalism, leadership, judgement, and accountability in interactions with client and families.
  • Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
  • Maintains documentation standards according to the College of Nurses of Ontario.
