Description:

An exciting opportunity has come up to work as rotational Band 5 physiotherapist for Ealing community Partners . The staff will have 6 monthly rotation in different service eg MSK , Pulmonary rehab and community Rehab teams. We are proud to say that our Pulmonary rehab Team won the team of the Year award and it is PRSAS accredited team.
Staff will benefit from an opportunity to develop knowledge and skills from other senior clinicians including advance clinical practitioners and first contact practitioners. Service is also committed in staff development by providing regular supervisions and CPD opportunities.
This is a full time permanent role and requires a valid HCPC Physiotherapy registration. Candidate must have a broad under graduate experience in core clinical areas and should have trained or worked in the NHS.

JOB SUMMARY:

Provide a high standard of physiotherapy service to patients referred to services in Ealing Community Partners eg Musculoskeletal outpatients, Pulmonary Rehabilitation Service, community Rehab, Womens Health etc..
Participate in the Service development Projects
Provide sound and appropriate clinical advice on the management of patients, carers and other health care professionals.
Work without direct supervision.
Provide support, guidance and training to technical Instructors, physiotherapy assistants and students.
To undertake a comprehensive assessment of patients including those with diverse or complex presentations.
To use clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.
Formulate and deliver an individual physiotherapy treatment programme based on evidence based practice and treatment options using clinical assessment, reasoning skills and a broad knowledge and understanding of treatment skills.
To accept responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time.
To carry out assessment and treatments of a range of musculoskeletal, respiratory, medical , neurological conditions with moderate physical effort on a daily basis.
To have basic knowledge assessment and management of Long term conditions
To carry out assessment and treatment of patients referred for group classes in the community sites and out patient clinics.
Where required support home visits
This role is an Ealing Community Partners role through CNWL.
Community-based health care in Ealing is delivered in partnership with West London NHS Trust, and Central and North West London NHS Trust, (CNWL) the Hillingdon Hospitals NHS Trust and some other smaller providers, including third sector. Together they have formed a partnership to provide integrated community health services in Ealing. The total value of the new 10 year contract is about £50m per year.
We have also committed to invest in improved links with local voluntary sector organisations (for example ECIL, Ealing Dementia Concern, Ealing Mind) and other community assets for innovation and flexibility.

To clarify and explain the patient’s condition to them to enable the formulation of realistic agreed treatment goals and plans. This may sometimes involve the breaking of unwelcome news to the patient and/or their carers.
To deal sensitively with patients who have high levels of anxiety and depression as part of their complex presentation.
To deal sensitively with the relatives and carers of patients.
To comply with the Trust’s Manual Handling policy and local therapeutic handling guidance at all times.
To be responsible for maintaining accurate and comprehensive patient treatment records in line with Trust procedures and Chartered Society of Physiotherapy (CSP) standards of practice.
To write concise, informative reports detailing treatment outcomes and levels of disability for medical staff, employers and solicitors.
To train, supervise and performance manages more junior staff including technical instructors.
To communicate effectively with patients and their parents, guardians or carers, to maximise recovery potential, and to ensure understanding of both conditions and treatment. Communication skills of persuasion, motivation and explanation to encourage patients to undertake their treatment programme.
To ensure that your own actions, and those of the staff you supervise, support equality, diversity and rights of our patients and their families.
To assess patient understanding of treatment proposals, gain valid consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
To maintain accurate record of patients’ attendances and treatments using Systmone
