Job Description:
Are you an experienced registered practical nurse looking for a different kind of practice environment, and comfortable practicing both independently and as part of a team? This could be what you’ve been looking for.
What will you do?
Working as part of the Information and Referral team, the RPN will provide information and appropriate referrals to both new callers as well as existing patients. This position will also provide a point of clinical escalation and determination of eligibility for home and community care services as required. The participation in the planning of care by collaborating with other multidisciplinary professional team members within the established scope of practice is critical.

Education / Experience / Knowledge

  • Registered Practical Nurse, in good standing with the College of Nurses of Ontario.
  • Good knowledge of primary care services and other community resources as well as the roles of health care professionals.
  • Experience working with various health disciplines as well as the public, interagency contact.
  • Understanding of the long-term care and health care delivery system in the province of Ontario.
  • In-depth knowledge of tools, systems and databases used in patient service delivery and management.
  • Advanced verbal proficiency in French an asset.
  • Other languages an asset

Responsibilities
  • Providing information and referral as appropriate to callers.
  • Completing the InterRAI Preliminary Screener to facilitate appropriate referrals.
  • Providing a point of escalation for clinical issues.
  • Practicing independently and interdependently within the established scope of practice.
  • Promoting accessible, equitable services by responding promptly to individual patient needs and service/care plans.
  • Promoting and planning for patient independence and self-management of chronic and complex issues.
    What must you have?
