Description Basic Function: To support the operations of the School Based Health Center Program. Principal Accountabilities: 1. Knowledge of all aspects of the SBHC Program. 2. Manage recall lists for needed dental appointments and schedule when due 3. Manage parental consent forms for services and send requests for updated documents when needed 4. Schedule follow up appointments including treatment, 6-month rechecks, and sealants as requested by the dental hygienist. 5. Send letters to parents regarding future appointments and seek updated consents when due 6. Communicate with school personnel to schedule meetings as needed 7. Track monthly and annual enrollment in the SBHC programs 8. Assist with completion of NYS Department of Health quarterly reporting 9. Prepare monthly Quality Management Committee reports 10. Prepare monthly reports for staff completion such as Well Child Checks 11. Answer incoming phone calls, transfer patients calls where needed and schedule appointments as appropriate 12. Represent the SBHC program at school open houses and other outreach events. 13. Assist with and promote all processes associated with the Coleman initiative 14. Promote Patient Centered Medical Home initiatives 15. Assist SBHC staff with all other tasks as assigned 16. Travel/provide coverage at various SBHCs as needed Requirements Knowledge/ Education Requirements: A. High school graduate or GED B. Medical Assistant or Certified Nursing Assistant training strongly preferred C. Experience with Microsoft Office products- Word, Excel, Outlook, PowerPoint D. Verbal and written communication skills, office applications preferred E. Demonstrate proper judgment and decision-making skills when necessary F. Comply with the organizations code of conduct, safety rules and adheres to all company policies G. Carry out job responsibilities in an ethical, effective and professional manner H. Must be willing and demonstrate the ability to cooperate, work, and communicate with coworkers and supervisors I. Must demonstrate computer proficiency with Microsoft Suite and electronic medical record system J. Must be keenly aware of the importance of confidentiality in all aspects of the position
