32 hours weekly, with 4 8-hour shifts - working every Monday and rotating weekends (1 weekend per month)

SCHEDULING COORDINATOR POSITION SUMMARY:

Under direct supervision of the Community Director, Area Director or Admissions/Clinical Manager, the Scheduling Coordinator helps ensure referral and/or current patient visits are scheduled effectively and efficiently based on patient/family need and appropriate Hosparus Health team member availability. This individual is responsible for coordinating the scheduling of referral and/or current patient visits and assisting in the maintenance of staff schedules for Hosparus Health community-focused admission and clinical teams. The Scheduling Coordinator will maintain Hosparus Health team member availability lists and serve as a point of contact for admission and clinical team members, patients, and various internal departments and other communities, as necessary, regarding the scheduling of referral and current patient visits.

SCHEDULING COORDINATOR POSITION QUALIFICATIONS:

• High school diploma required.
• 2 years of previous scheduling or related experience; healthcare scheduling experience preferred.
• Demonstrated software proficiency required, including knowledge of Microsoft Office Suite.
• Excellent organizational and problem-solving skills.
• Ability to assume responsibility, work independently, and demonstrate analytical judgment.
• Ability to multi-task and communicate effectively to varied audiences in a fast-paced environment.

• Coordinates with admission and clinical teams and various internal departments to schedule daily referral and current patient visits based on need within required time frames to ensure quality service.
• Analyzes and determines the best available resources to utilize based on patient, family and organizational needs, referral source needs, work schedules, patient/family cancellations, staff call-ins, vacation requests and unscheduled visits.
• Communicates clearly, concisely and in a timely manner with admission and interdisciplinary clinical teams, patients, families, and referral sources regarding scheduling of visits and/or other changes in schedule for admission and clinical team staff as appropriate.
• Collaborates across the organization as needed to assure coordination and continuity in delivery of care.
• Assists in verifying visit and/or mileage accuracy as needed for the purpose of ensuring visits are billed and mileage is reimbursed appropriately.
• Assigns referrals and/or new patients to communities/teams as directed.
• Keeps Clinical Managers & Community Director informed of caseloads, assignments, personnel needs and scheduling difficulties to ensure assignments provide efficient coverage.
• Assists Community Directors & Clinical Managers with staffing for Crisis Care.
• Coordinates schedules with admission and clinical staff regarding sick leave, vacations, and/or other types of leave; works with staff to accommodate changes in schedule and ensure coverage.
• Runs reports as necessary and maintains confidentiality of all data and information.
• Enters staff and patient information into the Electronic Documentation System (EDS) regarding conversations with patients, family, and staff regarding appointments, cancellations, patient/family preferences, etc.
• Participates in the evaluation of department functions, identifying trends and areas for improvement as appropriate.
