Description Position Summary The School-Based Oral Health Coordinator is responsible for planning, coordinating, and supporting school-based oral health programs and partnerships on behalf of High Country Community Health (HCCH). This position serves as the primary liaison between the health center, local school systems, community organizations, dental providers, and families to improve access to preventive and restorative oral healthcare services for children and adolescents. The coordinator oversees program operations, outreach, scheduling, compliance, reporting, and care coordination activities to ensure delivery of high-quality, patient-centered oral health services within school settings and affiliated community programs. This position is based in Boone, NC, but would coordinate for all the counties HCCH serves: Surry, Burke, Avery, and Watauga. Essential Duties and Responsibilities Program Coordination and Operations ? Coordinate daily operations of school-based oral health programs across assigned schools and community sites. ? Develop and maintain relationships with school administrators, nurses, counselors, teachers, and community stakeholders. ? Collaborate with dental leadership to implement preventive and restorative oral health initiatives aligned with HCCH and FQHC goals. ? Coordinate school screening events, sealant programs, fluoride varnish initiatives, and oral health education activities. ? Maintain program schedules, site calendars, and provider assignments for mobile or school-based dental services. ? Ensure appropriate consent forms, documentation, and patient records are completed and maintained. ? Maintains SDS, OSHA, and Radiation manuals for the mobile units. ? Keeps up to date with maintenance of dental equipment and coordinates mobile unit needs throughout departments, including but not limited to IT/EHR services, maintenance, and interpreter services when needed. Patient Outreach and Care Coordination ? Conduct outreach to students and families regarding available oral health services. ? Assist families with appointment scheduling, insurance enrollment, referrals and follow- up care. ? Educate patients, parents, and school staff on oral hygiene, preventive care, and available community resources. ? Track referrals for restorative treatment and specialty dental services. ? Coordinate continuity of care between school-based services and HCCH clinics. Compliance and Documentation ? Ensure compliance with HRSA, HIPAA, OSHA, Radiation and state dental regulations, and school district requirements. ? Maintain accurate program records, patient data, productivity reports, and grant-related documentation. ? Support quality improvement initiatives and performance reporting for school-based oral health programs. ? Assist with preparation of reports for grants, contracts, and organizational leadership. Community Engagement and Partnerships ? Represent HCCH at school meetings, community events, health fairs, and coalition activities. ? Build partnerships with community organizations to support oral health access and education. ? Participate in strategic planning efforts related to pediatric oral health and school-based care expansion. Administrative Support ? Monitor inventory and supply needs for school-based dental programs. ? Assist with onboarding and coordination of dental staff, students, or volunteers participating in school-based activities. ? Support grant implementation and special projects as assigned. ? Other roles as assigned by the dental leadership team. Requirements Education ? An Associates or Bachelor’s degree in Public Health, Healthcare Administration, Social Work, Education, Dental Assisting, or related field is preferred. ? An equivalent combination of education and experience may be considered. Experience ? Minimum of 1 year of experience in healthcare coordination, community health, school health programs, dental services, or public health preferred. ? Experience working in an FQHC, public health setting, or school-based program is strongly preferred. ? Experience with pediatric populations and underserved communities is preferred. Knowledge, Skills, and Abilities ? Knowledge of oral health practices, preventive dental care, and community health principles. ? Understanding of HCCH operations, HRSA requirements, and care coordination models preferred. ? Ability to maintain confidentiality and comply with HIPAA requirements. ? Strong organizational, communication, and relationship-building skills are required. ? Ability to work independently and manage multiple school sites and priorities. ? Proficiency in electronic health records (EHR) systems (especially Epic), Microsoft Office, and Google applications. ? Bilingual English/Spanish preferred. Physical Requirements ? Ability to travel between schools, community sites, and HCCH locations. ? Ability to lift and transport program supplies and educational materials up to 25 pounds. ? Ability to work occasional evening or weekend community events as needed. Work Environment This position operates in a combination of school settings, community environments, mobile dental units, and clinical office settings.
