Description

This post is an exciting opportunity within Humber and North Yorkshire Integrated Care Board. The post holder will review and case manage patients that are subject to Section 117 aftercare (Mental Health Act 1983) commissioning and people with learning disabilities and/or Autism ensuring that the services commissioned are appropriate and safe to manage service user care. The post holder will work independently and meet with the multidisciplinary professionals, service users and carers within the service and ensure clear understanding of case management and programme of care needs on a continuous basis, discuss potential transfers to alternative placements with service users when necessary. This includes people living in the community and who are in Specialist placements including in and out of area placements.
The post holder would be a resource to support service providers with knowledge of alternative placements and treatment options. Case manage patients who are subject to s117 aftercare and transforming care. To ensure all placements provide appropriate, good quality services and are value for money. Liaise with Legal team regarding DoLs/CoP enquires Re-assess all new patient placements within initial 12 weeks (as per local s117 Aftercare Policy) assessment period to provide highly specialised advice and ensure placement suitability and discuss with service user quality and care issues.
Manage potentially highly complex, sensitive and contentious service user information during interactions with legal representatives and independent provider clinical teams where discussions may be complex and require a calm, balanced approach to resolving conflict. Attend service users clinical/multiagency reviews and, if appropriate, provide advice with complex Section 117 aftercare and Care and Treatment Reviews (CTR) pre-discharge meetings to ascertain appropriateness of placements, working with provider clinicians and ensure least restrictive option. Attend Mental Health Review Tribunals (MHRT) when required to provide update report(s) on behalf of the ICB
