Responsibilities

JOB PURPOSE:

Assists persons supported to develop their Self-Direction budget while also utilizing a circle of support and a person-centered approach. A consistent caseload will be maintained, the size of which is dictated by program management. The broker is expected to work with other members of the person supported Circle of Support (e.g., family, case managers, etc.) to ensure satisfaction with services as well as health and safety. The Broker is required to maintain a flexible schedule to attend meetings (reliable transportation is a must) outside of normal business hours, per the schedule of persons supported and their designees.

Key Tasks: Broker

  • Follow and adhere to all regulatory guidelines as published by the New York State Office for People with Developmental Disabilities (OPWDD) and Medicaid.
  • Establish and maintain Broker Authorization requirements for Support Brokers per NYS Administrative Memoranda (includes 12 hours of annual training (re)certifications).
  • Fulfill duties as outlined in the Broker Agreement for a person supported, including, but not limited to:
  • Assist person with start- up and support broker services; includes developing the initial Self-Direction budget.
  • Monitor Self-Direction budget to include yearly rollover of budget and additional changes needed throughout the year.
  • Assist person supported with recruiting, hiring, training, and managing their self-hired staff.
  • Complete Staff Action Plans in accordance with NYS Administrative Memoranda.
  • Complete all documentation as required per the NYS Administrative Memoranda for Service Documentation for Support Broker Services.
  • Attend and facilitate, when applicable, all Circle of Support meetings for the person supported.
  • Attend and participate in the person supported Life Plan meetings 2x/year.
  • Maintain a consistent weekly and monthly billing standard for all persons on the caseload.
  • Other duties as assigned.
