About the role: As Senior Case Worker, this is your opportunity to be the backbone of our daily service delivery— supporting efficient day to day service delivery and ensure that case workers provide case management, assessment and referral, education, and support in accordance with TSA and service guidelines. Provide support to the team ensuring services are provided within our Model of Care. This is a full-time permanent role based in Nowra, NSW. Salary is based on the SCHADS Award, Level 5. How you will make an impact Deliver high-quality, person-centred services through education, interventions, group work, assessments, referrals, and case management. Develop and monitor individual treatment plans, maintaining accurate documentation and data in line with case planning procedures. Champion continuous improvement and innovation in service delivery. Oversee daily operations to ensure efficient and effective service provision. Maintain up-to-date knowledge of referral pathways and support options for clients. Collaborate professionally with internal and external services. Promote active service user participation and feedback. Provide line management and supervision to Case Workers. Foster strong, productive relationships within the team. Embed reflective practices to enhance client outcomes. Build team capability through coaching, mentoring, and training. Support team wellbeing and ensure a safe working environment. Assist with recruitment in partnership with the AOD Team Leader. Address performance and conduct matters with support from management. Represent the service through presentations to local organisations and stakeholders. Establish and maintain strong referral pathways to support positive client outcomes. Participate in sector and local area network meetings as required.
