JOB SUMMARY: Provide operational and case management to assigned residential locations.
EDUCATION OR TRAINING REQUIREMENT: Bachelor’s Degree in Social Service such as Psychology, Social Work, Sociology or related field. Must meet GA Developmental Disability Professional (DDP) requirements.
EXPERIENCE REQUIREMENT: 5 years of related experience and 5 years of supervisory experience
SPECIAL LICENSES, REGISTRATION, CERTIFICATION, OR SKILLS: GA Driver’s License and vehicle insurance; must be proficient with Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)
SUPERVISORY RESPONSIBILITY: Supervises Community living managers

Responsibilities

RESPONSIBILITIES RELATING TO HEALTH AND SAFETY

  • Submit completed site visit tool bi-monthly to Community Living Quality Coordinator which ensures that home meets all safety requirements and identify quality concerns.
  • Manages plans for potential emergencies and disasters, such as medical, fire, severe weather and missing persons. Discusses with Individual’s Support team specific role relating to each emergency.
  • Approves GER’ and submits CIR’s and Safety Plans per UCP and DBHDD policies.
  • Weekly review of Support Coordinator Ratings and coaching, referrals should be resolved in 15 days.
  • Submission of Corrective Action Plans (CAP) when required. Responsible for ensuring all CAP related actions items are submitted by due date.

CASE MANAGEMENT RESPONSIBILITIES:

  • Runs and audits biweekly reports of all Therap documentation to include but is not limited to shift notes, ISP goal tracking and bowel tracking to ensure they are present and content is thorough and accurate.
  • Ensure transportation and supervision for residents as dictated by individual program.
  • Reviews the completed person-centered calendar each month which supports the implementation of activities as designed by the individual’s support team.
  • Assess the needs of individuals and identify any required services, such as equipment, clothing, or other essentials.
  • Complete bi-monthly DDP Notes to ensure accurate documentation of individual progress and family contact is completed. Visit homes as noted in Therap Case Note and document the date and time of each visit
  • Conduct Nursing Assessments to evaluate and address the health and medical needs of individuals.
  • Participate in all SIS meetings
  • Participate in and complete all ISP (Individual Support Plan) Meetings, ensuring individual goals and objectives are met.

Financial Responsibilities

  • Ensures Region remains in compliance with budgets (staffing, food, household supplies, etc). Conducts budget reviews on a quarterly/monthly basis.
  • Review for accuracy and submit monthly Petty cash and CTF reimbursement along with reconciliation form by due date to Director of Community Living.
  • Ensure individuals CTF’s do not exceed 2000.00 in any given month and authorize funds added to house credit card for individual purchases
