Reporting to the Director of Clinical Transformation within the Population Health and Value-based Health Systems portfolio, the Business Lead will help to establish a vision for Remote Care Management (RCM) with specific objectives that align with the Ministry’s Health Data and Digital Strategy Refresh, OH’s Annual Business Plan, and goals of the Population Health and Value-based Care Portfolio. The Business Lead will play a key role in the development of a 5-year strategic implementation plan that addresses how OH will scale and spread high impact RCM programs, ensuring that these programs are embedded into clinical pathways and are easily accessible and available for all Ontarians. The ideal candidate will have a clinical background (preferably a certified health practitioner) or other relevant experience in the delivery of clinical programs, together with advanced knowledge and experience of the Ontario healthcare system including primary, acute, and home care sectors, and virtual health care delivery. This position involves developing and maintaining relationships with multiple, diverse stakeholders at all levels. The stakeholders include digital experts, government, vendors, researchers, funders, regional health system planners, clinicians and administrators. Experience in clinical program management, technology implementation and management, change management, business development and entrepreneurial skills are essential for this role.

Here is what you will be doing:

  • Work closely with external stakeholders and the OH teams within the Population Health and Value-based Health Systems portfolio to develop a strategy and implementation plan that drives quality and standardized remote care management models that are embedded in routine healthcare to improve system capacity, value, access and outcomes for all Ontarians. This includes the ability to:
  • Manage complex relationships across the healthcare sector, including OH Regions, OHT’s, primary care, healthcare providers, the Ministry and other OH stakeholders.
  • Conduct comprehensive engagements with partners to identify barriers, opportunities and inform strategic decisions that will improve system and patient outcomes.
  • Support the development of the 5-year RCM strategy and implementation plan, including project plans and budget allocation.
  • Work to align the RCM provincial model with other clinical programs, such as integrated clinical pathways, centralized intake, OHTs, ALC initiatives, etc.
  • Identify emerging issues in the community and execute strategic responses.
  • Bring expertise in the implementation, clinical adoption, change management and evaluation of RCM models of care.
  • Work with the OH team to monitor the implementation to ensure ongoing quality improvement of the RCM provincial model.
  • Coordinate and conduct analyses of local health-care system based on provincial data, community input and other sources for purpose of identifying gaps and establishing planning priorities.
  • Promote a culture of quality through the adoption of high-quality research and planning methodologies.
  • Conduct effective stakeholder consultation and engagement related to planning, assesses impact of planning recommendations and decisions, and promote consensus-building among stakeholders.
  • Participate in and contribute to provincial planning activities as well as conduct effective stakeholder consultation related to planning, assessing impact of planning recommendations and decisions.
  • Manage innovative virtual care projects and lead cross-functional internal and external teams which will include any combination of strategy, marketing, communications, procurement, clinical innovation, operations, government, funders, product development, privacy, security and evaluation/analytics.
  • Leverage and incorporate existing benefits evaluation models focused on access, quality and integration to assess the health system outcomes associated with target models of care.
  • Develop strategy, specific plans and proposals for OH projects and programs and collaborate on the development of the tactical deployment plans.
  • Provide professional expertise, knowledge and strategic advice to OH executive sponsors and staff to ensure a coordinated approach to primary care access & navigation related activities across OH regional & provincial teams in response to complex issues and risks requiring strategic and evidence-based recommendations.
  • Provide strategic advice to executive sponsors or sponsor groups on complex multi-stakeholder, multi-disciplinary change initiatives.
  • Develop and present new ideas and conceptualize new approaches and solutions.
  • Utilize project and change management principles and monitor performance through regular tracking of key metrics.
  • Perform other related duties as assigned by the manager.

Here is what you will need to be successful:

Education and Experience

  • Registered health care professional or related bachelor’s degree in the health field, Healthcare Informatics or Business Administration.
  • Master’s degree in Health Administration, Health Informatics, or other healthcare-related graduate degree preferred.
  • Experience in delivering digital/virtual care (as a clinician or administrator) is strongly preferred.
  • Experience in developing strategies and execution of those strategies for adoption and uptake of virtual care.
  • Experience in overseeing large, complex projects involving the delivery of health care and technology.
  • Minimum 10 years’ experience in health setting, and/or as a health technology vendor or consultant with evidence of thorough understanding of health care system and system stakeholders.
  • Experience and formal training combined with demonstrated performance and ability may substitute for stipulated academic/experience requirements.

Knowledge and Skills

  • Knowledge of emerging issues, challenges and gaps in ensuring equitable and effective health care across Ontario, particularly in the primary, acute, and community care sectors.
  • Knowledge of Ontario Health’s legislated stakeholders and their priorities, requirements for effective engagement
  • Relationship management skills to develop and manage key relationships critical to furthering Ontario Health’s mandate.
  • Technology/service vendor relationship management and complex contract management skills
  • Self-starter with outstanding customer service ethic, strong leadership and change management skills.
  • Experience developing and/or executing “big picture” strategic plans.
  • A sophisticated stakeholder relationship manager with a passion for transforming access to health care including Researchers, government/regional administrators and healthcare clinicians.
  • Demonstrated success in implementing innovative technologies in a clinical environment, delivering results, on time and on budget.
  • Solid understanding of principles and implementation of value-based care.
  • Effective communicator, able to communicate effectively and concisely, both orally and in writing, including effective presentation skills.
  • Skilled in collaboration and developing strategic partnerships.
  • Excellent problem-solving skills, organizational, project management, decision making and analytical skills.
  • Able to take risk and be creative and innovative.
