  • A Fixed-Term, Full-Time opportunity to work with a high-performing and passionate team in the Victorian Public Service until 30th June 2028
  • VPS Grade 5.1: $113,022 - $124,885 plus superannuation.
Responsibilities

The Senior Case Managers play a key role in the service delivery of the therapeutic schemes, providing frontline services to participants. Their role is to:

  • Apply evidence-based practice in the case management of participants, ensuring effective assessment, planning, intervention, and review
  • Identify, assess, and evaluate factors that relate to an individual’s behaviour and provide individualised risk assessment, case planning, service coordination, deliver effective interventions to reduce risk, and liaise with suitable stakeholders for the delivery of appropriate interventions,
  • Motivate and influence participants in the case management process. Model pro-social behaviours and draw on strengths-based approaches,
  • Monitor participant’s compliance against case management plans; demonstrate assertiveness and confidence in dealing with participants exhibiting challenging behaviours and escalating issues to senior staff in a timely manner,
  • Present on the progress of an individual’s participation in the therapeutic schemes to CVE MAP members and other stakeholders,
  • Proactively build and maintain positive relationships with key strategic internal and external stakeholders such as service providers within a highly sensitive operating environment to support the ongoing effective delivery of therapeutic interventions to vulnerable or at risk individuals, and
  • Maintain a commitment to the highest standards of confidentiality and professional integrity in relation to all matters of both schemes.
