About the Role Rapid Rehousing for Young Adults is a solution to homelessness designed to help young adults aged 18-24 quickly exit homelessness to permanent housing. The program provides rental assistance and support services that are tailored to the unique needs of the young person. The senior case manager will provide case management to priority clients. Senior case manager will also provide coaching and training to case managers. Senior case manager will assist program supervisor as needed. What You’ll Do · Complete & Co-facilitate intake assessments. · Evaluate the needs of the client, both immediate and on-going needs. · Maintain a caseload of up to 7 Priority clients · Conduct Monthly Home Visits with Priority Clients & Document Case Progress Notes. · Develop individualized service plans for each youth and update. · Provide coaching to case managers. Train new case managers in best practices and procedures. · Ensure all participants have a budget plan that is updated at least quarterly. · Assist the supervisor with charts, documents and notes review. · Ensure Case managers are making appropriate internal and external referrals · Collaborate with SW at least bi-weekly to discuss Client’s with MH concerns. · Conduct Home Visits with case managers at least quarterly for 50% of housed participants · Document Case Management Progress Notes. · Lead intakes, participate in treatment team meetings and discharge planning meetings. · Provide crisis intervention to youth in their apartments and mediate roommate conflicts · Conduct outreach and presentations as needed and as assigned · Collaborate within an interdisciplinary team Your Qualifications · Bachelor’s degree from an accredited college. · 2-5 years of experience of working with adolescents. · Knowledge and understanding of the homeless population, homeless youth a plus · Comfortable traveling throughout NYC. · 2 years of Rapid Rehousing Case management experience preferred Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities required of the employee for this job. Days and hours of work are Monday through Friday in Queens, NY. Work hours options are 9:00 am - 5:00 pm, 9:30 am - 5:30 pm, 10 am - 6 pm.
