Provide individual and group therapy to program participants Develop and implement individualized plans of care Participate in interdisciplinary treatment planning Assist patients with aftercare and discharge planning for community support Ongoing participation in program development Participate in Quality Management activities and initiatives Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Education Master's Degree from accredited school of Counseling OR Master's Degree from an accredited school of Social Work Work Experience 5+ years in provision of chemical dependency services, including: partial hospitalization, intensive outpatient programming, aftercare services, group and individual therapy (Required) and Prior outpatient chemical dependency experience (Required) and Prior experience in program and educational development (Required) and Prior Supervisory experience (Required) Knowledge, Skills, & Abilities Familiarity with community support agency resources (Required proficiency) Licenses and Certifications Certification as CCDC-IIIE by Ohio Department of Alcohol and Drug Addiction Services, eligibility for licensing as LICDC. (Required) Licensed Social Worker (LSW or LISW) in the State of Ohio OR Licensed Professional Counselor (LPC or LPCC) in the State of Ohio Position Details/Preferred Qualifications Must have at least 5 years of experience of chemical dependency services, including partial hospitalization, intensive outpatient programming, aftercare services, group and individual therapy in an outpatient environment. In addition, previous work in program and educational development as well as prior supervisory experience is also required. This is an outpatient setting caring for adult patients. Interest and/or experience in working with young adults (age 18 - 25) is strongly preferred. This is a day shift position but does require a minimum of one or two evenings per week: 1:00pm - 9:00pm. Must hold the CCDC IIIE certification by Ohio Department of Alcohol and Drug Addiction Services and be eligible for licensing as a LICDC.
