The primary goal of this homeless outreach service is to extend healthcare support to hostels and street homeless residents within the London Borough of Brent. The role focuses on improving clients’ health and well-being through various activities, including:
Health Promotion: Educating clients on healthy living practices.
General Health Screening: Conducting routine health checks to identify and address health issues early.
Engagement in Primary Care and Drug and Alcohol Services: Facilitating access to organised healthcare services.
Develop pathways into specialist services for clients.
Train hostel and partnership outreach staff to manage residents outside regular hours.
Conduct nurse-led general health clinics within hostels and for street homeless individuals.
Support clients, particularly those with alcohol dependency, where traditional treatment methods may not be suitable.
