REQUIRED QUALIFICATIONS:

Bachelor’s Degree in PHARMACY from an accredited School of Pharmacy Or
PharmD in PHARMACY from an accredited School of Pharmacy
Postgraduate Year One (PGY-1) Pharmacy Residency Or equivalent educ/experience
Upon hire: Oregon Pharmacist License
Upon hire: Pharmacy preceptor license in applicable state as applicable
3 years Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy Residency

PREFERRED QUALIFICATIONS:

Within 2 years of hire: Board Certified by the Board of Pharmacy Specialties specific to area of practice within 2 years of starting the role
