We believe our people make the difference.   About the role    As a Senior Clinician within the Community Rapid Triage Team you will work autonomously, but be well supported by your colleagues, assessing clients following their discharge from an Eastern Health hospital who have been referred to the Community Rehabilitation Program. You will assist with rapid triage, assessment, goal setting and discharge planning for clients. You will recognise the deteriorating patient and escalate to medical staff for management to prevent avoidable readmissions to hospital. Partake in holistic discharge planning following inpatient stay, including liaising with community nursing, Post-Acute Care, HARP coordinators and allied health.     This is an excellent opportunity to progress your career in a supportive environment with regular professional development.     About you   To be successful in this role you will possess:    Demonstrated expertise in managing complex and vulnerable, frail populations, initiating appropriate therapies and supportive treatments.   Demonstrate a high level of clinical reasoning and advanced casework skills, using a patient-centred approach within multi-dimensional assessments, formulating appropriately targeted treatment plans with realistic achievable goals.   Develop and implement treatment plans in collaboration with patients and significant carers.   Collaboration with other stakeholders and especially referrers, CRP and Post-Acute Care teams.   Demonstrated ability to participate in innovative practice and quality improvement.   Deliver a high quality, accessible and coordinated service to our clients both in the client’s home and in the centre.   Be an active member of the interdisciplinary team.   Demonstrate high-level organisational skills through efficient time management and professional responsibility including modelling the ability to appropriately prioritise workload and manage multiple competing demands.     Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice).
