Position Overview In this role, you will: Provide individualized therapy services to aging adults within assigned senior communities Support, mentor, and collaborate with therapy teams across multiple sites Serve as the primary contact for facility administrators and partners Help strengthen relationships and promote Greenfield Rehab’s mission and values Work closely with the Regional Director to ensure clinical excellence, team satisfaction, and smooth daily operations This is a high-impact, hybrid clinical/leadership opportunity ideal for a PT or PTA who enjoys autonomy, communication, and building strong care environments for seniors. Qualifications Licensed Physical Therapist or Physical Therapist Assistant in the State of Wisconsin Experience in senior care settings (ALF, SNF, Memory Care, or Home Health) strongly preferred Strong communication, leadership, and organizational skills Ability to support multiple communities and foster positive relationships Minimum two years of experience preferred (PT or PTA), but strong candidates with less will be considered Why Choose Greenfield Rehab?
