Job Description   Serves as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Maintains a caseload of members and provides various support services by conducting assessments and in home visits and promoting well-being of care to members. Serves as a mentor and works alongside team members to provide and implement various care activities for members.  Job Responsibility   1.Participates and contributes to member's appointments and meetings to support member. 2.Conducts chart audits to ensure quality compliance and adherence to processes. 3.May facilitate Team meetings and 1:1's in the absence of Supervisor. 4.Serves as a liaison between the Supervisor and Community Health Workers (CHW). 5.Breaks down and communicates reports to staff. 6.Assists with the onboarding and training of new hires. 7.Assist with the design and implementation of workflows. 8.Serves as a mentor and acts as a resource for CHW. 9.Provides oversight in care of members to improve health outcomes and promote well-being. 10.Provides support and basic health and education about material and general health care and community resources.  11.Conducts one-on-one health education, community outreach, regular in-home visits and service coordination. 12.Provides various supports, services, entitlements and benefits for members. 13.Supports members in developing health management care plans. 14.Empowers members by promoting effective approaches, problem solving and lifestyle changes. 15.Works to establish positive and support relationships and effective works with a diverse group of members.  16.Delivers coaching and mentorship on chronic health conditions and self-care. 17.Enters client data into software on a daily basis. 18.Advocates for members and their families and collaborates with the care team as needed. 19.Perform all related duties, as required. Job Qualification   •Associate’s Degree or equivalent combination of education and related experience. •3-5 years of relevant experience, preferred.
