Overview About the Role The primary purpose of the Senior Community Health Worker will play a pivotal role in coordinating and overseeing Community Health initiatives to promote the overall health and well-being of our University Hospital patients. This position requires a strong commitment to community engagement, program coordination, and collaboration with various stakeholders. The Senior CHW is an integral member of the multi-disciplinary team that works to endorse clinical teams to the community. Provides perspectives to enhance care management strategies while efficiently navigating community resources to provide patients with access and education on the importance of preventive care screenings and services. Responsibilities What You’ll Do Serves as a key leader in coordinating and overseeing Community Health initiatives that support the overall health and well‑being of University Hospital patients. Demonstrates a strong commitment to community engagement, effective program coordination, and collaboration with multiple internal and external partners. Functions as an essential member of a multidisciplinary team, helping bridge clinical services with community needs. Provides valuable insight to strengthen care management approaches and support clinical teams. Efficiently navigates community resources to connect patients with appropriate services, preventive screenings, and health education. Promotes patient access to preventive care and emphasizes the importance of maintaining long‑term health and wellness. Qualifications What You’ll Bring High school diploma or equivalent required; an Associate’s degree is preferred. NJ Department of Health CLG Community Health Worker Certification required, or must be obtained within six (6) months of hire at University Hospital. Minimum of three (3) years of recent experience working as a Community Health Worker. Experience participating in community organizations and familiarity with issues such as interpersonal violence, health disparities, and challenges common in inner‑city communities is preferred. Prior completion of a Community Health Worker training program is a plus. Preference for candidates who demonstrate sensitivity when working in environments impacted by substance use. Must be dependable, a strong and attentive listener, and able to build solid relationships with the public. Must be adaptable to the changing needs of the community. Ability to use an electronic medical record (EMR) system and document patient interactions is required. Must show a genuine interest in serving the Newark community or similar urban communities. Must be capable of working independently, seeking guidance when appropriate, and collaborating effectively within a team. Must be available for flexible scheduling, including evenings and weekends, to support community engagement and in‑person outreach. Must be able to engage with individuals from diverse backgrounds. Spanish or bilingual language skills are preferred.
