GENERAL REQUIREMENTS:

· Experience within the Health & Social Care setting.
· Full UK Driving Licence and access to road vehicle as the role is community based.
· A compassionate and empathetic approach towards people.
· To be trustworthy, honest and reliable.
· Excellent leadership and communication skills.
· Strong organisational skills with the ability to priorities effectively.
· Ability to work as part of a team and independently, using initiative when required.
· Basic I.T. skills i.e. using electronic recording systems, sending emails, virtual meetings, word processing reports and records.
· Importantly - you need to be kind, caring and ready to lead a team to ensure that our services users are safe and fulfilling their goals & aspirations.

QUALIFICATION REQUIREMENTS:

· Although not essential as training will be provided, QCF (Qualifications and Credit Framework) Level 2 or 3 Diploma in Health and Social Care or equivalent would be desirable.

Responsibilities

ROLE OVERVIEW:

As a Senior Community Support Worker, you will play a vital role in leading and co-ordinating a team of dedicated support staff. Your primary responsibility will be to ensure the smooth and effective delivery of support services to our service users. This includes supervising and mentoring support staff, conducting assessments, developing care plans and liaising with other professionals and external agencies.

RESPONSIBILITIES:

· Lead and motivate a team of support staff to provide high-quality care and support services to vulnerable individuals within the community.
· Helping our service users to live independently in their own homes by means of - administering medication - supporting with budgeting, shopping and meal preparation - assisting with activities of daily living including domestic duties - social activities and general companionship.
· Conduct assessments and develop person-centred care plans tailored to meet the unique needs of each service user.
· Ensure the implementation of care plans, monitoring progress, and making necessary adjustments as required.
· Liaise with external agencies, professionals and family members to ensure a holistic approach to care and support.
· Support the Registered Manager in managing and monitoring performance.
· Manage team workload as authorised by the Registered Manager, by delegating appropriate tasks to staff, directing and supporting them to organise and co-ordinate their specific tasks deadlines and quality standards.
· To maintain Health & Social Care skills at a current level and undertake training and development to maintain current practice.
· Ensure records of support provision are accurately maintained.
· Conduct Quality Assurance checks on service delivery standards, service user environments, records, service user finances and service user medications.
· To take an active lead in meeting organisational objectives.
· Provide guidance to support staff, conducting regular supervisions, appraisals and identifying any training needs.
· Maintain accurate and up-to-date documentation and records in compliance with company policies and regulatory requirements.
· Participate in the On-Call personnel procedure.
· Hold regular meetings with co-senior workers and Registered Manager and Directors.
· Promote a culture of respect, dignity, and empowerment within the team and foster positive relationships with service users, staff and external partners.
