    Our client, an organization in the public health sector, is seeking for a passionate and suitable candidate to join the team!

Job Responsibilities:

  • Build partnerships with community care providers (e.g., GPs) to deliver accessible and affordable care.
  • Develop and maintain relationships with partners to understand strengths, challenges, and resident healthcare needs.
  • Connect internal and external resources to enhance GPs’ capacity and capabilities.
  • Plan and implement collaboration programmes (e.g., shared care) to integrate primary and tertiary care for better patient outcomes.
  • Improve service access for primary care partners.
  • Implement quality improvement frameworks aligned with Healthier SG preventive care goals.
  • Identify improvement opportunities and facilitate collaboration platforms for learning, best practices, and training.
  • Drive continuous process review and improvement of assigned programmes and projects to meet objectives.

JOB REQUIREMENTS

  • Minimum Degree holder in any related discipline.
  • At least 2-3 years of experience in community care/ marketing/ business; preferably in a healthcare setting
  • Proficient in MS Office
  • Good interpersonal and communication skills with a passion for service
