Responsibilities

YOUR SPECIFIC RESPONSIBILITIES:

This is an intermediate level professional position required to perform a full range of assignments under the general supervision of the Health Services Manager A-SES in the Community Programs section. The primary responsibilities include mobilizing partners to address health gaps among underserved populations at a higher risk. This position is responsible for building community capacity that includes traditional public health organizations and nontraditional partners to reach disproportionately affected populations with tailored programs and practices for healthcare strategies.
Convene and facilitate a multi-sector community health coalition that includes members of underserved communities and organizations that serve the community. Build cross-sectional partnerships to align public health healthcare and non-health interventions that decrease health gaps. Engage partners to fulfill health improvement plan activities by facilitating meetings, collecting data, reporting on progress, assisting with project tasks, and conducting accountability follow-ups to implement priority health area projects. Coordinate presentations to coalition members, updating them on the projects’ progress, issues encountered, partner recruitment, and data tracking.
Coordinate/participate in activities/events that support the improvement of resident’s health status.
Support County Health Department (CHD) activities and outreach to increase access to health-related services to populations that are underserved. Assist with the development of all foundational plans. Ensure all foundational plans include an optimal health focus.
Maintain a CHD team consisting of staff from various program areas. In collaboration with the team, administer and promote an assessment to identify professional opportunities that support the implementation of priority health area projects, as needed.
Facilitate training opportunities for County Health Department staff and other community partners to increase their capacity to address health priority areas.
Oversee administration and reporting of county community health work plan and budget. Coordinate any contracted services related to the health improvement plan. Track priority area projects goals, objectives, activities and update data quarterly. Provide information as requested.
Disseminate information monthly to the appropriate partners regarding training, resources, events, grant opportunities, health care access and quality, best practices, etc. Identify grant opportunities. Maintain documentation of requested and disseminated resources.
Serve as a back-up for information security and incident reporting duties.
Attend professional development and training.
Other duties assigned.
