Job Summary:
We are seeking a caring and experienced Healthcare Assistant to join our dedicated team. The successful candidate will be responsible for supporting the delivery of high-quality care to our residents in a home care setting. The ideal candidate will have a strong background in healthcare, excellent communication skills, and a passion for delivering person-centered care.
Responsibilities:
- Assist healthcare professionals in providing personal care and support to residents in their homes, care homes or nursing homes.
- Develop and implement personalized care plans that meet the individual needs of each resident.
- Provide assistance with daily living activities, such as bathing, dressing, and grooming.
- Monitor and record vital signs and other health metrics as required.
- Support with medication administration and management.
- Communicate effectively with residents, families, and healthcare professionals to ensure continuity of care.
- Utilize IT systems to maintain accurate records and communicate with the healthcare team.
- Maintain a clean and safe environment for residents and staff.
Experience:
- Previous experience working in a healthcare setting, preferably in home care or a care home.
- Experience with care planning and delivery.
- Proven ability to communicate effectively with residents, families, and healthcare professionals.
- Ability to drive to and from residents’ homes and care homes.
- Basic knowledge of IT systems and software.
- Fluency in English is essential for effective communication with residents and healthcare professionals.
- Ability to work independently and as part of a team.
- A genuine passion for delivering person-centered care to vulnerable individuals.
- Experience in Practical Moving and Handling of Persons
- Experience in Providing Person- Centered Care and Promote independence for Service Users
Skills:
